        The Initial Sign Up Fee of $450 covers:

	Entry of each of your clients into the database
	A Resource Notebook of important information
	Guidance in establishing and/or growing your practice

       Our On-going Fee:

	12% of what you get paid from insurance companies
	Discounted rates for high Medicaid claim volumes

       Long-term contract not required
